WHAT IS PROJECT CARE

Project Care is an initiative to identify and rectify challenges across the social care sector. Our primary focus has been creating and supporting strong relationships at the frontline between recipients of care and their care workers, with an aim of reducing care worker shortages and enabling person-centered care that truly meets the needs of the recipient.

To facilitate the formation of these bonds, we have also targeted continuity within the care system, endeavoring to provide clarity and guidance, reducing the time from enquiry to accessing the appropriate care services.

OUR VIRTUAL ASSISTANT

Our Virtual assistant is trained to handle any request related to social care, guiding users through the care system, providing access to accurate, up-to-date information and resources relevant to their specific context.

They will remember any previous chats, ensuring information doesn't fall through the cracks; provide real-time, up-to-date information from official sources and your profile; and provide plain-language conversation to make it easy to understand.

LIFEBOOK PRO

EMPOWERED, EFFECTIVE CARE TEAMS

For Care Workers and Providers, Lifebook PRO supports management of the ongoing care delivery, with a primary focus on empowered, self-organising care teams providing person-centered care.

We support the management and access of in-depth client care information; digital Care Diaries to keep other Care Workers and networks on the same plus client and team management, including rota, scheduling, and communication channels.

ORCHESTRATION

GETTING THE RIGHT PEOPLE, THE RIGHT CARE, AT THE RIGHT TIME

We also understand that creating the right relationships is sometimes the largest barrier. That's why we're also creating and orchestration service to bring both halves together, connecting care requests (whether from the Lifebook App directly, or LAs / ICSs) and channelling them to the right providers via Lifebook PRO.

CHANGING THE FACE OF SOCIAL CARE

Through the Lifebook Platform, we will change the way people experience the Adult Social Care system, from the orchestration and planning level of Local Authorities, ICS's and Care Providers down to the front line of Recipient of Care and Care Worker relationships. We will provide:
